Privacy notice for the Patient Journey Planner
Version 1.0 — test environment template. This notice must be reviewed and completed by Meditona’s responsible legal and data-protection team before production launch.
Purpose
The planner helps a potential patient organise practical preferences and request contact about international healthcare coordination. It does not provide medical advice, diagnosis or treatment eligibility decisions.
Information collected
The tool may collect contact details, country/city, communication preference, broad healthcare interest, planning preferences, requested support, source/partner attribution and technical security information. It does not request medical records or detailed symptoms in the public form.
How information is used
Information is used to respond to the request, coordinate the appropriate next contact, protect the service from abuse and maintain referral attribution where applicable.
Health-related information
Some answers may indicate a broad area of healthcare interest. Submitted records are encrypted at rest in the test implementation. Full answers are not sent through ordinary email or Telegram notifications. Medical records should only be transferred later through an approved secure process.
Legal basis and retention
The production privacy notice must identify the relevant Meditona legal entity, lawful basis, retention periods, international transfers, processors and data-subject rights for each target market.
Your choices
Submitting the contact form is optional. You may use the planning result without submitting a request. Contact the Meditona data-protection contact listed on the official website to request access, correction or deletion where applicable.
